Abstract

MacIntyre NR, Cook DJ, Ely EW, Epstein SK et al. (2002) Evidence-based guidelines for weaning and discontinuing ventilatory support. Respiratory Care 47: 69-90

Patients require mechanical ventilatory support when the ventilatory and/or gas exchange capabilities of their respiratory system fail. As the conditions that warranted placing the patient on the ventilator stabilise and begin to resolve, attention should be placed on removing the ventilator as quickly as possible (weaning).

This guidance addresses the followijg issues:

  • the pathophysiology of ventilator dependence
  • the criteria for identifying patients who are capable of ventilator discontinuation
  • ventilator management strategies to maximize discontinuation potential
  • the role of tracheotomy
  • the role of long-term facilities
