This guide is aimed at helping practitioners provide a seamless transfer for adolescents from paediatric to adult health care systems. It is divided into two parts: an overview of the issues to consider whilst planning transition services and a practical framework for working with young people at each stage of adolescence: early stage (around 12-14 years); middle stage (around 14-15 years); and late stage (around 15-16 years). There is also an appendix with diagram showing the RCN clinical pathway for adolescent transition.

This document was revised in 2013 to reflect a number of then-current political and professional issues and initiatives.
