A step by step guide for GP practices : annual health checks for people with a learning disability

Abstract

This guide is produced to help GPs, practice nurses and primary administration team organise and perform quality annual health checks on adults with a learning disability.

It focuses on the Cardiff Health check for People with a Learning Disability developed by Professor M. Kerr, Welsh Centre for Learning Disabilities.

The introduction of annual health checks with people with learning disabilities is important for the following reasons:

  • To improve health outcomes for people with learning disabilities
  • To help identify and treat medical conditions early
  • To screen for health issues particular to people with LD and specific conditions
  • To improve access to generic health promotion in people with LD
  • To develop relationships with GPs, practice nurses and primary care staff particularly after comprehensive paediatric care finishes at the age of 18.
