Developing trusted assessment schemes: 'essential elements'

This guide describes how hospitals, primary and community care and local councils could work together to implement trusted assessment to effectively and safely reduce the numbers and waiting times of people awaiting discharge from hospital. It sets out core criteria that need to be in place for all models of trusted assessment and a suggested checklist that local systems can consider when developing their local schemes. This includes that all participating organisations should co-design and agree the trusted assessor profile and the process for the trusted assessor to follow. Trusted assessment is a key part of the High Impact Change Model, a national condition for funding through the 2017-19 Integration and Better Care Fund policy framework. The guide has been developed across NHS England, NHS Improvement, the Local Government Association, the Association of Directors of Adult Social Services, the Care Provider Alliance and the Department of Health.
