Abstract

Psychosocial factors have an important role in predicting unscheduled care in people with long-term conditions, alongside prior use, and a targeted patient intervention in primary care has shown high levels of acceptability.

Programme Grants for Applied Research (PGfAR) publishes evidence aimed at improving health outcomes in England with practical application for the benefit of patients and the NHS in the relatively near future. Find out more about the PGfAR journal.
